Customer includes Recorded Number (RP-####-######) on plans submitted to OCE for design drawing approval.

The actual Recorded Easement documentation needs to be uploaded with the drawings for approval.

- Form 584 - General Utility Easement (PDF or Word)
- Proof of Ownership/Deed
- Survey (review the checklist for required survey items)
- Metes and Bounds
- Email all five (5) required PDF documents (300 dpi or greater) to [email protected].
- Staff will provide an email notice to schedule an appointment for "Easement Dedication Submission/Assistance"
- On the day of your appointment, bring the original Form 584 (with wet signature in BLUE INK ONLY) to 1002 Washington Ave, second floor.
